Heart Failure Center

Heart failure is one of the leading causes of death in the United States and is the number one cause of hospitalization in patients over 65 years of age. Unlike other cardiovascular disorders, the instances of heart failure are increasing every year. More that 5 million Americans are currently being treated for heart failure and another 500,000 are diagnosed each year.  But, with proper treatment and disease management, patients can now lead longer, more fulfilling lives. Covenant Heart & Vascular Institute’s Heart Failure Center is dedicated to helping people with heart failure manage their conditions and have a better quality of life.

 

The Outpatient Heart Failure Center provides a comprehensive, caring program for heart failure patients in an outpatient setting. We believe this complex disease is best managed with a team approach, and every patient receives a personalized program that may involve:

  • Physician
  • Nurses
  • Dietician
  • Pharmacist
  • Rehabilitation Therapist
  • Case manager
  • Spiritual Care

 

We understand that every patient is unique not only in terms of his or her cause and course of heart failure but also in terms of his or her personal and cultural approach to the disease. Our team will develop a treament plan unique to each patient that can include the following options:

 

  • Intensive education
  • Encouragement for patient to be aggressive and involved in their own care
  • Telephone follow-up
  • Careful review of medications to improve adherence to evidence based guidelines
  • Lipid management
  • Point-of-care testing
  • Individualized clinic follow-up
  • Intermittent vasoactive and diuretic therapy by ACLS trained nurses
  • Monitored ACE Inhibitor and Beta Blocker initiation and titration
  • Electrolyte monitoring and replacement

Any physician may refer patients to the Center with a Physician Order. The referring physician will maintain control of care and will receive results of all lab tests and procedures performed.  A “visit note” will be added to the patient’s record which includes a brief assessment, current medications, diuretic therapy and the medication titration schedule.

 

Our patients have measured success of a decreased number of repeat hospitalizations, better results on a quality of life survey and a patient satisfaction survey.
